In cPanel when you go to Subdomains, you can easily create them. Then you can go to name.domain.com or domain.com/name.

On the next question, do you mean you want name.domain.com/folder ? If you password protect the folder "name" then as far as I know, you won't be able to access /folder without putting in the password for the entire subdomain. Give it a try and see what happens.. post back here if you need to.
