josie,
I think you are looking at the wrong set of DNS settings. You do not want the DNS section of your control panel at Surpass, you want the control panel of your domain at 123-reg.co.uk. There you will find a section for changing nameservers for your domain. In that section you will enter the corresponding ns988.dizinc.com and the other one from your welcome e-mail. You should not be seeing A records CNAME. Those are server settings that were automatically configured when your account was set up and need no further modifications.

The IP would be the same as what you received in your welcome email
you can also use ftp.domain.com

I've tried every IP address I can find. Nothing works. It seems to work and then something goes wrong. Here's a screenshot:
