New Domain Name: cpadental.com
Self explainitory, it is the domain you wish to add.
Quote:
Username/directory/subdomain Name: ???????????
What you enter here will be the name of the username (used for FTP, mail, etc), directory where the domain points to (within the /public_html/ folder), and the subdomain (it'll auto add the .domain.com of your main domain)

Password: ??????????
The password, for such things as the addon domain's own FTP and email accounts.

----------------

Now your issue is that the domain is already added onto the server's DNS records, meaning it could have been...
  • Added by another client (for any reason).
  • Is currently parked
  • Is currently added.
  • or Another reason
Please check your Addon domain section to see if it is added already, or check the parked domains. If those do no show any sign, please contact support as this means, for whatever reason,

Also, make sure (if reseller account) the domain isn't already added in its own account in your WHM... Or (if shared) isn't on its own account.

Best to just contact support if this troubleshooting doesnt work for you.
